For dental and vision

  • Zip Code
  • Current plan
  • Current election tier

For disability, life, vol life and worksite

  • Job title
  • Salary
  • Elected volume (for voluntary benefit)
  • Benefit class (if applicable - e.g. management, staff, etc.)
